     OFA and PennHip Certification appointments are scheduled on Dr Ireland surgery days only. Your pet is dropped off in the morning between 7:30am - 8am, stays for the day, and is picked up after 5pm that evening. This procedure does require anesthesia and pre-op bloodwork, so your pet must be fasted the night before the procedure. They must also be up to date on all vaccines.
 
    The following information is needed  for completion of certification:
   1. Permanent ID - Can be in the form of a microchip or tattoo. If the dog does
       not have either, it can be done at the time of the procedure.
   2. AKC Registration number
   3. Sire and Dam information if available
   4. Date of Birth (must be 2 years old for permanent OFA)
 
     OFA - The owner gets the xrays and form.
               The owner mails them.
               The owner pays for the certification.
 
     PennHip - The owner signs the form.
                      The hospital mails the xrays.
                     The hospital pays the certification fee.
                     Owner receives a copy of the OFA views and form to submit
                       if they want.
